WAIVER AND TERMS & CONDITIONS

By clicking “Accept,” I confirm that I am the parent or legal guardian of the participating minor and that I am legally authorized to provide consent on their behalf. In consideration of my child’s participation in programs offered by The Camp Provider, I agree to the following terms:

1. Assumption of Risk and Release of Liability I acknowledge that participation in holiday camps, sports activities, and related programs may involve physical exertion and risk of injury. I certify that my child is in good health and physically capable of participating in all scheduled activities. I hereby waive, release, and discharge The Camp Provider, its staff, affiliates, volunteers, and any partnering or hosting organizations from any and all claims, demands, or liabilities—whether known or unknown—arising out of or related to any injury, loss, or damage sustained during the course of the camp. 
2. Medical Authorization In the event that my child becomes ill or is injured during their participation in the camp, I authorize The Camp Provider’s staff to administer appropriate first aid and, if necessary, seek further medical attention, including emergency medical services. I agree to bear any costs associated with such treatment. 
3. Health & Safety Disclosure I agree to inform The Camp Provider of any pre-existing medical conditions, allergies, injuries, or psychological factors that may affect my child’s ability to safely participate in camp activities. I understand that failure to disclose such information may increase the risk of harm and relieve The Camp Provider of any associated liability. 
4. Agreement to Terms By accepting these Terms & Conditions, I acknowledge that I have read, understood, and voluntarily agree to be bound by all the provisions above. I also understand that this agreement shall be governed by the applicable laws of the jurisdiction in which the camp takes place.
